HEALTH PROMOTION COORDINATOR

Perth Family Health Team’s (PFHT) mission is to provide accessible, collaborative, and compassionate healthcare to our community. Reporting to the Interprofessional Healthcare Providers (IHP) Manager, the Health Promotion Coordinator plays an integral role in connecting the community with healthcare. The Health Promotion Coordinator coordinates the development, implementation, and evaluation of health education and promotion programs and activities across the lifespan. As a member of the interprofessional health team, the Health Promotion Coordinator will support the integration of health promotion strategies across primary care and allied health services at PFHT. The Health Promotion Coordinator champions evidence-based planning in programming and is instrumental at local, regional, and provincial levels with respect to new directions.

Perth Family Health Team is seeking an innovative and enthusiastic individual to join our team with strong skills and attributes in communication, program planning, group facilitation, data analysis and research, health education, and community engagement. A bachelor’s degree in health promotion or in health sciences, public administration, recreation administration, hospital administration, social science or related field is required.
