At Transitions, we strive to give every youth the opportunity to become healthy, productive and make informed choices for their future.
Our target population is youth between 12-17 years of age, in the Ottawa Valley and surrounding areas.
Our Group Home’s primary responsibility is the youth and their well-being. We advocate for each youth’s health and best interests by providing them with the tools to make appropriate decisions; To make accountability a requirement, opposed to an option. With a combination of effort from both the youth and the staff at Transitions, growth, development, and success is inevitable.
