| Job Description | The Hospice Hub is recruiting for a Registered Nurse/Registered Practical Nurse to fill the part-time position (approx. 21 hours per week) one year term position of Nurse Coordinator, Home - Support. The Nurse Coordinator provides clinical oversight of The Hospice Hub’s Home Support Program. The successful candidate will plan, coordinate, deliver, and support a team to provide responsive, end of life care for clients and their families.
