The Counselor will meet clients individually to provide assessment and treatment interventions and ensure the most appropriate interventions are offered. They will participate in case conferences and contribute to treatment planning and case management. The Counsellor will facilitate psychoeducational groups and provide supportive counselling. The Counsellor will assist clients in navigating the health system with respect to referrals to other agencies, completion of applications and paperwork, and advocating for services. All services are provided in accordance with the mission and established protocols of the Agency.

This position is based within the HART Hub, a 66-bed, two-site initiative providing a full continuum of addiction and mental health services across Lanark, Leeds, and Grenville counties. This role is specifically located within our 8-bed residential addiction treatment program, which is co-located with 12 supportive housing spaces. The HART Hub is designed to break down traditional service silos, integrating withdrawal management, intensive treatment, supportive treatment, and long-term housing to provide client-centered, continuous care.
